Muslims in America
Leadership Questionnaire
For pioneers, founders of mosques and Islamic institutions, organizational leaders, imams, professionals, businesspeople, educators, activists, women and youth leaders.
Assalaamu Alaikum,
Purpose of This Questionnaire
A research-based book, tentatively titled Muslims in America, is being written by Zaheer Uddin, Executive Director of Center for American Muslim Research and Information (CAMRI) and former President of ICNA and Internet Islamic University, to document the history, development, institutions, contributions, achievements, challenges, and changing experiences of Muslims in America.
This questionnaire has two purposes. First, it seeks to recover historical information and personal experiences that might otherwise be forgotten. Second, it seeks thoughtful evaluation of the development of Muslim community life in America.
If you'd like to share more details, we can provide a more comprehensive questionnaire.
Recognition of Your Contribution
If information supplied by you is used in the book, your contribution will be appropriately acknowledged unless you request anonymity. Particularly important firsthand information may be attributed to you in the text or notes, subject to the permission you provide at the end of this questionnaire.
Your candid, thoughtful, and critical assessment is particularly valuable. The purpose is not to promote or criticize any individual organization or viewpoint, but to develop an informed and balanced assessment of the achievements, shortcomings, challenges, and future of Muslim community life in the United States.
Your memory today may become a historical source tomorrow.
